Objective
Until the causes of workplace accidents and ill health are understood and managed, improvements will not be made in the protection
of employees from work activity hazards. Employers already have a duty to investigate and report a number of such matters and that
duty is being extended to cover all workplace accidents, occurences of work related ill health etc.

All incidents must be investigated to determine the underlying causes and identify failures of occupational health & safety
management. When action is taken to address each of the factors found as a result of investigation, not only is the likelihood of a
similar incident minimised, it may be instrumental in preventing accidents of a different nature in which one of these factors may play
a part.
